What is it and why is it important?
Advance Care Planning helps prepare individuals for future healthcare needs and end of life care. It's a way to help you think about and discuss your medical wishes with your loved ones, doctors and future healthcare agents.
It's important to start the discussion before you become too ill. Planning now will give you the peace of mind of knowing that your wishes are being carried out. It also eases the burden of your loved ones who often are left to make healthcare decisions for you if you are unable to communicate your wishes.
